Benevolence Activity Form - submit two copies for any charitable activity to the Human Concerns committee at least 30 days before your event.

Scholarship Form - for students attending Lutheran Colleges.  Due by July 1

Simply Giving Enrollment Form - print this form and give it to Jim Bruckman to enroll in Simply Giving,

Scrip Order Form - print this form to fill in your scrip order then just turn it in on Sunday morning.

Purchase Notification - to be used by committees and members making  purchases for All Saints.  Print this form and submit it to our bookkeeper.

Facility Use Policy Letter & Form - information about reserving our facility for outside events and an Authorized Use Agreement.
